IHPN and Rexanto are responding to a CMS RFP. CMS has offered $1 billion dollars in grant money to entities (groups of stakeholders) that can demonstrate innovative ways to lower cost and improve care. CMS has specifically mentioned high cost populations like seniors. IHPN and Rexanto are proposing to bring LTC best practices to high-risk seniors at home. The program will offer the following:

A short cycle solution either 7 or 14 days to Part D or dual recipients.

An in home monitoring solution that will enable the independent pharmacist to assess medication adherence.

A monthly review by a pharmacist of each patient’s drug regimen.
IHPN will offer training, P&P manuals, and CGP certification training to a pharmacist involved at each pharmacy. Rexanto will support the program by offering a central fill solution for the short cycle medications and monitoring devices. Our financial proposal includes compensation to the pharmacy for short cycle dispensing in the homes (we will utilize the current LTC pay structure for this), compensation for staff training, compensation for additional staff (driver, technician etc.). The final draft of the proposal document is due January 27th.
